U.S. Citizenship and Immigration Services (USCIS) is seeking public comment on its proposal to extend, without any changes, the information collection requirements for the Application for Civil Surgeon Designation, known as Form I-910. The agency published a 30-day notice in the Federal Register on July 1, 2026, inviting comments through August 6, 2026.
The notice follows a 60-day comment period previously announced in the Federal Register on April 20, 2026, during which USCIS received one comment, according to the agency. The current request is submitted to the Office of Management and Budget (OMB) for review and clearance under the Paperwork Reduction Act.
Form I-910 is used by physicians seeking designation as a civil surgeon authorized to complete the medical examination required for certain immigration benefit applicants. USCIS reviews the application to determine whether a physician meets statutory and regulatory requirements, including holding a valid medical license and having no adverse actions taken by state or territorial licensing authorities.
If approved, the physician is added to USCIS's public Civil Surgeon locator and may complete Form I-693 for applicants adjusting status to lawful permanent residence.
The agency estimates that 470 physicians will submit Form I-910 annually, with each response taking approximately two hours. The total annual hour burden is estimated at 2 hours, and the total annual cost burden is estimated at $24,205.
USCIS is asking commenters to address four specific points: whether the collection is necessary for the agency's functions and has practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the information collected; and methods to minimize the burden on respondents, including through electronic submission.
Comments must be submitted via the federal eRulemaking portal at www.regulations.gov under docket ID USCIS-2013-0002, with the OMB control number 1615-0114 included in the submission. All comments will be posted publicly on the portal.
